DETERMINING THE SEROPREVALANCE OF HEPATITIS B VIRUS AND SOME DEMOGRAFIC FEATURES OF PATIENTS WHO WERE ADJUSTED ADNAN MENDERES UNIVERSITY Aim:In our study, it was aimed that frequency of HBeAg,anti-HBe, HBV-DNA and Delta Anticor; assign some demografic features, correlation of using cigarettes and alcohol and some systemic diseases like diabetes mellitus, hypertrigliceridemia, chronic hearth disease, obesity in patients with HBsAg positive, inactive carrier, chronic active hepatitis and liver cirrhosis which HBV cause. Method:In our study, 603 patients with HBsAg positive; inactive carrier (n=214), chronic active hepatitis (n=254) and liver cirrhosis which HBV cause (n=135) who were admitted to Adnan Menderes University Gastroenterology/Hepatology clinic between the dates of May 2015 and May 2017 were included. Average age was 49.1±14.7 (19-88). %42.5 of patients were females, %57.5 were males. Data was obtained retrospectively and it was studied that seroprevalance and relationship between demografic features, use of alcohol, smoking and some systemic disease. Results:%35,5 of patients was inactive carrier, %42.1 was chronic active hepatitis and %22.4 was liver cirrhosis. HBeAg in %17.1, Anti-HBe %82.8 and Delta Anticor in %13.7 of persons were positive.Anti-HBs positivity was detected in %4 of patients in clinical course.Seroconversion ratio of anti-HBs was found as %2.79 in inactive HBV carriers yearly. HBV-DNA level was under 2000 iu/ml in%36.2 of persons. HBeAg positivity ratio was in %0.5 in inactive carriers, %30 in cronic hepatitispatients and %19.3 in liver cirrhosis. Anti-HBe positivity ratio was in %98,1 in inactive carriers, %70.2 in cronic hepatitis patients and %82.1 in liver cirrhosis. Delta Anticor positivity ratio was in %7.1 in inactive carriers, %10.4 in cronic hepatitis patients and %25 in liver cirrhosis. HBV-DNA levels was under 2000iu/ml in all inactive carriers while above 2000 iu/ml in all liver cirrhosis patients and %98.4 of chronic hepatitis patients. Diabetes mellitus was seen higher in liver cirrhosis that statistically significant. %33.3 of all patients were alcohol drinker, cirrhosis was seen much more in heavy alcohol drinker. The rate of smokers was%40.8 when all the patients in our study were taken. This rate was%36.5 in chronic hepatitis patients and %46.6 in liver cirrhosis. Conclusion:Alcohol , DM and HBV serology is very important parameters while searching and followingHBsAg positive persons. Anti-HBe positivity ratio is %82,8 in HBsAg positive persons in our region. HBeAg positivity is evidently higher in chronic active hepatitis and cirrhosis patients. Advanced age, male gender, DM are independent risc factors for progressing to cirrhosis. Progressing cirrhosis from chronic active hepatits takes approximately ten years. So this parameters should be evaluated in the diagnosing and following of HBsAg positive persons.
