The evaluation of newborn well-being with fetal doppler ultrasonography results in HİGH-risk pregnancy

Abstract (EN)

Middle cerebral artery (MCA) and umbilical artery (UA) Doppler waveforms and cerebroplacental ratio (CPR) measured by Doppler ultrasonography (USG) are increasingly recognized as an important assessment tool in identifying fetuses at risk. The aim of the study is to investigate whether there is a relationship between fetal Doppler USG measurements in the antenatal period and newborn well-being. This study was carried out with 100 high-risk pregnant women who applied to the Gynecology and Obstetrics Clinic of Adıyaman University Training and Research Hospital between 01.03.2020-01.03.2021. The variables used in the study consisted of age, height, body weight, obstetric history, fetal Doppler USG findings, birth characteristics, and infant characteristics. Percentiles and multiples of median (MoM) values were calculated for Doppler USG parameters. While 29 (29.0%) of the pregnant women were primigravid, the median gravida was 3. 34 (34.0%) of the pregnant women were nulliparous, 19 (19.0%) primipara and 47 (47%) multipara. The body weight of 34 (34.0%) newborn babies was below the 5th percentile, 19 of them (19.0%) were between the 5th-9th percentile, 20 (20.0%) were between the 10th-90th percentile, and 27 of them were above the 90th percentile. The UA-PI values of 50 (50.0%) pregnants is between 10-90th percentile, OSA-PI values of 57 (57.0%) women are between 10-90th percentile, CPR values of 45 (45.0%) women are between 10-90th percentile. NST was abnormal in 26 (26.0%) women and amniotic fluid stained with meconium in 3 (3.0%) women. Babies of obese women and preterm babies have lower APGAR scores at 1st and 5th minutes compared to babies of normal weight women and term babies. Pregnant women with normal NST have better OSA-PI percentile values, OSA-PI MoM and SPO percentile values compared to those with abnormal NST. UA-PI percentile values and UA-PI XVI MoM values of women with baby body weight in the 10-90th percentile are lower than those with <5th percentile. Fetal Doppler USG parameters and SPO provide valuable outcomes in the prediction of fetal growth, pregnancy process and pregnancy-related risk factors as a simple, inexpensive, non-invasive and reproducible method. We think that the use of these measures in clinical practice will be beneficial for a better postnatal well-being of the newborn.
