Evaluation of prognostic factors in secondary lung tumors and results of metastasectomy

Abstract (EN)
Altough secondary tumors of the lung is a part of systemic metastasis of primary tumor, they have to be analysed in a different category with their properties. Metastasectomy provides an improved survey in suitable secondary lung tumor cases. In this study we report prognostic factors and supplement of metastasectomy to the surveySixty cases who were operated for secondary lung tumor in Gazi Medical Faculty Department of Thoracic Surgery between June 2003-June 20098 were included to this study. All of the patients were evaluated for age, sexuality, specialities of primary and secondary tumor, surgical procedures. All of the patients were examined with their laboratory, radiologic and complementary systemic diseases. Prognostic factors and disease free interval were evaluatedOf the patients 33(%55) were male 27(%45) were female and mean age was 53. Histopathologic findings of the patients 42 were carcinoma, 12 were sarcoma, 3 were germ cell tumor, 3 were malign melanoma. The most frequent primary tumors were adenocarcinoma and renal cell carcinoma. There had been a difference between the number of nodules established radiologically and peroperatively. Surgical incision was prepared to the localisation, number and side of the nodules. All of the nodules were exiced with surgerySurvey was 59 months in carcinomas, 46 months in sarcomas, 47 months in malignant melanoma. Of the 28 patients were stage 2, 25 were stage 3 and 7 were stage 1. Of the 2 patients had wound infection. Morbidity was %3,4. Of the 1 patient had respiratory insufficiency and died. Mortality was %1,7As a result; if primary tumor controlled, extrapulmonary metastasis was not found, all of the nodules were resectable then metastasectomy is safe and effectual management in secondary lung tumors. Metastasectomy extends survey. Number of the nodules established with radiologically is less than peroperative nodules. Surgeon must exice all of the nodules which were established radiologically and investigate additional nodules intraoperatively.Important prognostic factors are tumor histopathology, disease free interval, number of metastasis and probability of additional chemotheraphy management.
