Abstract (EN)
Coronary artery disease is the leading cause of death in Turkey and the world. The high mortality and morbidity due to acute coronary syndromes makes the evaluation of this patient group even more significant in the emergency departments. In this study, the value of ultra-sensitive troponin to determine all cause mortality in the emergency department is studied. This study was carried out retrospectively by examining the patient records who presented to the Emergency Department of Akdeniz University Hospital between July 1, 2018 and June 30, 2019. Patients whose troponin levels were oredered with an initial diagnosis of acute coronary syndrome and whose values were in the range of 0.06 - 0.10 ng/mL were included in the study. The patients included in the study were screened from the National Death Reporting System and the patients who died were identified. During the study period, 1029 troponin tests were analized for 591 patients. The results of the first troponin were included in the study. The average age was 64.51±15.55 years and 368 of these patients were men (62.26%), 223 were women (37.73%). It is found that 168 (28.43%) of the patients died and 51 of them were discharged from the emergency department. A statistically significant relationship was determined between death and diabetes mellitus (p< 0.022). According to the regression analysis applied for the study group; the history of diabetes mellitus, hypertension and hyperlipidemia was found significant in mortality. Ultra-sensitive troponin level is effective in determining mortality in patients in the emergency department. Studies for lower thresholds of troponin will increase the sensitivity.
