Abstract (EN)
Introductıon : Pain is the most complaint within primary knee OA patients. This pain is usually classified as nociceptive pain. However , the weak relationship between radiographic findings and the patient complaints indicates a multifactorial nature of this pain. In OA , several studies showed that neuropathıc pain may occur. The purpose of this study was to investigate the fequency of neuropathic pain in patients with pimary knee OA and to evaluate the relationship of such pain with functional status , quality of life and depression. Materials and Methods : one hundred fifty patients over 40 years of age who were admitted to Adıyaman University Department of Physical Medicine and Rehabilitation with knee pain and who were diagnosed with knee OA according to the ACR criteria. S-LANSS(Self-administered Leeds Assessment of Neuropathic Symptoms and Signs) scales were used as neuropathıc pain screening questionnaires. Pain status was assessed by VAS(Visual Analogue Scale) , functional status was assessed by WOMAC(Western Ontario and Mc MasterUniversities Osteoarthritis Index) , quality of life was assessed by SF-36(Short form-36) and depression status was assessed by BDI(Beck Depression Scale). Results : of 150 patients , 117(%78) were female and 33(%22) were male. Average age was 61,71±8,48 , average BMI was 28,72±2,95. According to S-LANSS scales , 55(%36.7) of patients had neuropathic pain component. There was significant difference between the groups with neuropathic pain and without neuropathıc pain in terms of age. Also , . There was significant difference between the groups with neuropathic pain and without neuropathıc pain in terms of symptom duration and morning stiffnes duration. There was significant difference between the groups with neuropathic pain and without neuropathıc pain in terms of VAS , WOMAC , BDI scales and all SF-36 parameter scores. İn our results , there was a positive correlation between BDI scores and S-LANSS , VAS, WOMAC scales. Also , there was a negative correlation between all SF-36 parameter scores and S-LANSS , VAS and WOMAC scales. Conclusion : İn OA patients , neuropathic pain is also the reason of the pain. Patients with neuropathic pain have higher knee pain severity and higher depression scores,lower functional status and quality of life. Neuropathic pain should also be considered in the treatment of primary knee OA. Key words : Knee , osteoarthritis , neuropathıc pain , WOMAC, SF-36 , depression
