Abstract (EN)
Hospital and community-acquired infections play an important role as a factor in Gram-negative bacteria resistant to antibiotics is increasing. One of the most common of these resistors which-ESBL-producing enzyme is β-lactamase. ESBL-producing microorganisms, not just the last ten years of hospital-acquired infections in community-acquired infections began to take place among the important factors. Community-acquired infections in the majority of ESBL positive Escherichia coli (E.coli) as a factor of urinary tract infections (UTI) constitute.ESBL positive microorganisms community-aquired UTI increase in the incidence of oral empirical limits their treatment options, increases the cost of treatment options are not appropriate. From the studies of patients affected with this infection mostly patients with underlying risk factors that have been identified. In this study, ESBL-producing E. coli in urinary tract infections caused by ESBL-producing ESBL determine risk factors that lead to the production and the associated mortality and morbidity minimized take measures to ensure the protection and control is intended. Between April 2012- April 2013 our outpatient clinic applicants and urine cultures E.coli with color reproduction and community-acquired urinary tract infection in 103 patients included in the study meets the criteria, ESBL - positive E. coli infection case group patients, ESBL -negative E. coli infection cases were included in the control group. E.coli isolated from patients'demographic characteristics, hospitalization, urine culture was sent to clinics, previous history of hospitalization, underlying diseases, immunosuppressive therapy retrieval operations, assets, invasive interventions (urinary catheter, cystoscopy, colonoscopy), broad-spectrum antibiotics, Charlson comorbidity scores were evaluated as risk factors. The two groups were compared in terms of risk factors. In patients with metastatic solid tumors (15.4% vs. 2%), moderate-to- severe renal disease (26.9% versus 9.8%), dementia (5.8% vs. 0%) There are significant differences (p < 0.05). Urinary history of surgery in the case of urinary tract infection development in case group 26.9% in the control group 9.8% and statistically significant as was observed (p < 0.025). Complicated U in those with score point higher as significant, respectively (p < 0.05). Recurrent UTI history in considering the case and control groups, respectively, 44.2% and 11.8%, respectively. This condition is statistically significant (p <0.001). 34.6% in the case of the use of antibiotics, has been found to be 11.8% in the control group was significant (p <0.05). Complicated UTI-uncomplicated UI that comes with patients evaluated Charlson score point average uncomplicated at 1.59, while at 2.4 bulunmuştur. Complicated UTI in those with score point higher as significant, respectively (p <0.05). Admitted to our hospital and ESBL-producing E. coli, caused by community-acquired urinary tract infections risk factors in determining our future treatment options will be identified and infection control measures in our study to be a guiding suggests. ESBLs CLSL does not recommend routine screening and be notified. However, this approach in our country's diagnosis, treatment and infection control practices we believe is not appropriate in terms. According to the results of our study, we ESBL-producing microorganisms in routine laboratories to investigate and suggest that clinicians should be reported.
