Inhaled iloprost effect on pulmonary hypertension

Abstract (EN)

Pulmonary hypertension is a progressively pathological process can occur seconderly a lot of cardiopulmonary disease and characterized by higher then 25 mmHg resting pulmonary artery pressure. The iloprost which known then last two decades is one of the therapeutic agents. The aim of this study is observing the efficiency the inhaled iloprost according to the functional classes of the patients. 45 adult patient was enrolled the study group who were treated with inhaled iloprost for pulmonary hypertension belongs to various etiologies in Hospital of Akdeniz University School of Medicine between 2012, December 01 and 2014, May 12. All patients who enrolled the study was separated as two groups according to the functional capacities (FC) and FC≤2 named as Group-1, FC>2 named as Group-2. All patients was followed via echocardiography on every 4 months and reobserved their FC's. There were no differences observed on pulmonary artery pressure between Group-1 and Group-2 in the end 4th month follow up echocardiography but Group-2 showed significantly upper decrease in FC. The end 8th month follow up echocardiograph showed that Group-1 is superior than Group-2 for decreasing pulmonary artery pressure but in FC decreasing, Group-2 showed significantly higher improvement. The end of the 12nd month follow up echocardiography showed that there are no differences between Group-1 and Group-2 for pulmonary artery pressure decreasing but improvement of the FC showed that inhaled iloprost therapy is more efficient in Group-2. According to the result of this study and recent guidelines, the chosen of inhaled iloprost as monotherapy agent in patients who has FC-3 and FC-4 is more efficient but in patients who has FC-1 and FC-2 should be chosen bosentan or sildenafil as monotherapy, can be more appropriate treatment option.
