Abstract (EN)

Aims: Lung cancer is the most common type of cancer worldwide. The most effective methods in the treatment of lung cancer is surgery. In centrally located tumors, sleeve lung resections are an effective and safe treatment method. Sleeve resections,as a parenchyma-sparing surgery method, provide positive contributions to patient comfort and survival in the perioperative period.A retrospective study was planned to evaluate our patients who underwent sleeve lung resection treatment in general. Materials and Methods: Thirty-five patients who underwent sleeve lung resection wtih the diagnosis of Non-Small Cell lung resection between 2014 and 2021 in the Department of Thoracic Surgery ,Faculty of Medicine,Karadeniz Technical University were included in the study. The data of the patients were determined retrospectively from their medical records. Data include age, gender, comorbidities, symptoms, smoking status, pulmonary function test results, tumor localization, methods used for preoperative tissue diagnosis, histopathological cell type, surgical resection type, pathological stage, nodal involvement, postoperative complications, adjuvant therapy types and mortality rate. Survival and factors affecting survival were evaluated statistically. Results: When the data of the patients were analyzed, 6 (%17.1) patients were Stage ІA, 3 (%8,6) patients were Stage IB , 2 (%5,7) patients were Stage IIA,19 (%54,3) patients were Stage IIB,4(%11,4) patiens Stage IIIB,1(%2,9) patient were stage IIIB. The most common type of sleeve lung resection is right upper sleeve lobectomy in 18(%51,4) patients, followed by right lower sleeve lobectomy in 5 (%14,3) and left lower sleeve lobectomy in 5(%14,3) patients. After pathological examinations, 22 (%62,9) patients had squamous cell carcinoma. In the postoperative period, chemotherapy (CT) was given to 11(%31,4) patients, and chemoradiotherapy (CRT) was given to 9 (%25,7) patients. In the survival analysis, 1-year survival was %85,7 and the median survival time was 1093 days. The mortality rate was %14,3. Conclusion: Sleeve resection performed in accordance with oncological principles are the resections that should preferred primary in all suitable case due to low morbidity and mortality, higher survival compared to an upper resection and clinical comfort. Keywords: Lung Cancer, Sleeve Resection, Paranchyma-sparing.
