Evaluation of coracoacromial ligament degeneration with magnetic resonance imaging

Abstract (EN)
Coracoacromial ligament (CAL) degeneration is a well-known indicator of subacromial impingement. The purpose of this study was to investigate the relationship CAL thickness on magnetic resonance imaging (MRI) with grade of CAL degeneration and types of rotator cuff tears. In addition, to investigate relationship the other anatomical measurements on magnetic resonance imaging with CAL degeneration and rotator cuff tear types and compare them with the literature. Patients who applied to Bolu Izzet Baysal Training and Research Hospital department of orthopedics and traumatology between January 2015 and December 2021 and underwent shoulder arthroscopy were retrospectively scanned. A total of 560 patients, 268 men and 292 women, were included in the study. CAL degeneration was classified according to the Royal Berkshire Hospital Classification by examining the arthroscopy images. In addition, patients were categorized according to the types of rotator cuff tears in arthroscopy. CAL degeneration was grade 0 in 142 patients, grade 1 in 186 patients, grade 2 in 217 patients and grade 3 in 15 patients. Decreasing mean CAL thickness was significantly associated with both of the increase in the degree of rotator cuff tear and increase grade of cal degeneration (p<0,001). This study revealed that as the grade of cal degeneration and the degree of rotator cuff tear increases, mean cal thickness decrease on mri. In addition, it is predicted that anatomical risk factors may be important for clinicians in determining the prognosis and treatment procedure in impingement syndrome.
