Abstract (EN)

INTRODUCTION AND PURPOSE: Obesity is defined as the excessive and abnormal accumulation of fat in the body. For the obese patients, the risk of hypertension, diabetes mellitus and cardiovascular system diseases increase and in conclusion the kidneys become the target organ. In the early phase, the marker that indicates the kidney?s being affected is that the increase in microalbumin in urine. The purpose of this study is to determine the frequency of microalbuminuria among the 40-year-old and over obese patients who applied for diagnosis and treatment in the first degree and to analyse the affecting factors.METHODOLOGY: The study is a cross-sectional analytical study. The sample consisted of 40-year-old and over obese patients whose body mass index (BMI) is 30 kg/m2 and over that applied to the health clinics in the centre of Malatya. Within the scope of the study, 116 male and 306 female, a total of 422 patients were examined. The participants? weight, BMI, body fat ratio and body fat mass were measured using the TANITA bioelectrical scale. In order to determine the hypertension and diabetes mellitus disease, the participants? blood pressure and random plasma glucose were measured. Nycocard Reader II was used for the measurement of microalbuminuria of the urine samples taken. SPSS 15.0 was used to analyse the data for this study. Chi-square Test, Mann-Whitney U Test, Kruskal-Wallis Test, Spearman?s rho correlation coefficient and logistic regression analysis were applied.RESULTS: The average age of the obese patients within this study was 52.9±10.9 years old. The frequency of microalbuminuria was found to be 31.5% and the frequency of macroalbuminuria was found to be 6.6%. Female participants had a rate of 32.7% microalbuminuria and 6.2% macroalbuminuria while male participants had 28.4% microalbuminuria and 7.8% macroalbuminuria and the difference was found to be insignificant. As the age increases, the frequency of macroalbuminuria increased as well. There are no meaningfull relationship between anthropometric measurements; body fat ratio, body fat mass, BMI, waist-hip ratio, arm circumference; and microalbumin levels. The probability of microalbuminuria in male patients with a waist circumference of 102 cm and over, and in female patients with a waist circumference of 88 cm and over increased (OR) 3.1 times (CI: 1.10-8.77) according to the ones having under these measurements. The presence of chronic diseases in obese patients was found to be an effective factor for microalbimunuria and macroalbuminuria. The probability of microalbuminuria increased (OR) 2.8 times (CI:1.79-4.56) in patients with a diastolic blood pressure of 90 mmHg and over according to the ones having under these measurements. The probability of microalbuminuria increased (OR) 3.2 times (CI:1.32-7.84) among the patients with a random plasma glucose level of 200 mg/lt and over when compared with the patients under 200 mg/lt of random plasma glucose level.CONCLUSION: Consequently, as the age increases the frequency of macroalbuminuria also increases. At the same time, hypertension and diabetes mellitus diseases together with obesity, increase the frequency of microalbuminuria and macroalbuminuria. For this reason, in services of primers health care with a rapid technices a regular monitoring of the microalbumin in urine for the patients with hypertension and diabetes mellitus disease can be suggested.
