Abstract (EN)
H.pylori is a transferable, microaerophilic, and gram negative bacterium with multiple virulence factors and infects more than 50% of world population. With mucosal colonization of H.pylori, the expression rates of cytokines and adipokines increase, leading to subclinical chronic inflammation and associated insulin resistance in peripheral tissues.Insulin resistance (IR) plays a role in the etiopathology of many diseases such as MS, diabetes mellitus, HT, and dyslipidemia. IR is calculated using HOMA formula where fasting glucose and blood insulin levels are used, and HOMA ?2.5 is considered to indicate resistance.Metabolic syndrome, a heterogeneous disease that develops on the basis of IR, is caused by genetic, infectious, or environmental factors. H.pylori is one of the infection agents that cause MS. Although its etiopathology is not clearly known, H.pylori infection increases the expression of proinflammatory cytokines, which damages the cytokine balance that in turn results in chronic subclinical inflammation. This leads to insulin resistance and MS formation.In our study, patients with metabolic syndrome were evaluated for H. pylori infection by histopathological diagnostic methods. Two hundred volunteer who applied to the outpatient clinic of Gazi University, Gastroenterology Department with dyspeptic complaints and had indications for endoscopic evaluation between April and October 2010 volunteered to participate in the study. The prevalence of H.pylori in general patient population is 49.5%. One hundred MS patients were compared with 100 non-MS patients, and the prevalence of H.pylori in the MS group was found to be 78%. No gender or age differences were found between H.pylori positive and negative groups. Moreover, using logistic regression analysis, H.pylori infection was shown to increase the risk of MS development by 13 folds. Among the MS criteria, the mean systolic-diastolic blood pressure and BMI of H.pylori positive patients were statistically significantly higher than those in H.pylori negative patients. Likewise, in H.pylori positive group, fasting glucose level, insulin and HOMA-IR, and VLDL and TG were statistically significantly higher, while HDL. Were no differences for TC and LDL.
