Abstract (EN)
Purpose; Obstructivesleepapneasyndrome (OSAS) is a disordercharacterizedwithrepetetiveapneaepisodesduringsleep.Periodiclimbmovementdisorder is repetetivemovementswhichoccurduringsleep. Thepurpose ofthisstudy is toevaluatethe presence of periodiclegmovementsduringsleep in OSAS patientswithusingpolysomnographicdata.Mostpatientsaredirectedtopolysomnographybased on pre-diagnosis of respiratuardisorder in sleep, but PLMD is oftendetectedaccidentallyandtheimportance has not yet beenunderstood. Materialandmethod; Inthisstudyweanalyzedpolysomnographyic data of1200 subjectswhowerehospitalizedbetween 2008 and 2015 withprediagnosis of obstructivesleepapneasyndrome in Karadeniz TechnicUniversity, Faculty of MedicineSleepUnite.1055 subjectswerediagnosed OSAS and 556 of 1055 patientsweredirectedto PSG examinationforsecond time. 146 subjectshad normal polysomnographyexaminationandweren'tdetected OSAS. Age andsexinformations of thesubjectswereobtainedfromarchiverecords. Results:OSAS group is olderthancontrolgroupstatistically. Inaddition,withtheincrease of diseaseseverity,agewasalsoincreased. Most of thepatients in OSAS groupweremale (%60,2). 1066 of 1200 subjects (%88,8)weredetected PLM index<10 and 134subjects(%11,2)weredetectedPLM index≥10. But withtheincrease of diseaseseveritytherewasnosignificantdifference in prevalanceof PLMocccurence.AfterCPAP titration PLM index has significantlyincreased in OSAS patients (p=0,000). WhenpolisomnograficdataswerecomparedwefoundthatREM latencywaslongedandsleepefficitywasdecreasedwiththeincrease of OSAS severity.Sleeplatencyand WASO wereshorter, sleepefficity,arousalindexand AHI werehigher, averageoxygensaturationwaslower, minimum oxygenpressure time waslonger, obstructiveand total apneaweremoreoften in patientswith PLM index<10. Conclusion; OSAS and PLMDaretwooftensleepdisorderswhicheffectsleepquality. Examining PLM withonlypolysomnographicdata is not sufficient in OSAS patients. Especially PLMDwhichoccursafter CPAP treatmentmust be watchedoutand itmust not be forgettenthat PLMDcouldaccompanywithmanyothersleepdisordersbesides OSAS.
