Comparison of oxidative stress situation and quality of life according to TYPE of treatment in patients with TYPE 2 diabetes mellitus

Abstract (EN)

Aim Diabetes Mellitus (DM) is an endocrine disorder that affects many systems and is characterized with high blood sugar. It is known that oxidative stress plays a role in the formation of early and late complications in the etiology of the disease. It has been shown that the quality of life is affected by the long-term course of the disease and the lifelong ongoing treatment duration of treatment for DM patients. Controlling the disease with appropriate treatment prevents the development of complications and improves the quality of life. The aim of this study was to compare the status of oxidative stress and quality of life according to the type of treatment in Type 2 DM patients. Methods Total 90 type 2 DM patients with no other disease except HT, between ages of 40-60 were included in to our study, who were applied to internal medicine outpatient clinics of Dumlupınar University Evliya Celebi Training and Research Hospital between May 2015 – November 2015 and among these 90 patients; 45 patients were using oral anti-diabetics (OAD) and 45 patients were taking insulin treatment. Venous blood samples were taken from patients in order to measure Total Antioxidant Status (TAS), Total Oxidant Status (TOS) and Paraoxonase-1 (PON-1) values in addition to their routine examinations. The SF-36 quality of life questionnaire was administered to patients. Statistical analyzes were performed using GraphPad version 6.05. Values of P <0.05 were considered statistically significant. Results In the group receiving insulin treatment, TOS and calculated OSI values were found to be statistically significantly higher than OAD group (p<0,001 and p<0,001). PON-1 averages of the OAD group were significantly higher than those of the insulin group (p:0.007). In the OAD group; Physical function, social functioning and pain subscale average scores of the SF-36 quality of life questionnaire, were significantly higher than that of insulin group (p<0,001, p:0.003 and p:0.003). In study patients; negative correlation was detected between TAS averages and FBG and HbA1C (r:-0.254, p:0.02 and r:-0.260, p:0.01), positive corelation was detected between TOS averages and HbA1C and triglyceride values (r: 0.225, p:0.03 and r: 0.225, p:0.03), positive correlation was detected between OSI values and FBG and HbA1C values (r:0.293, p:0.005 ve r:0.342, p:0.001). Conclusion When we compared diabetic patients receiving OAD and insulin therapy in two groups; it was found that the oxidative stress parameters were higher and the quality of life was worse in the group receiving insulin treatment than the OAD group. We conclude that these results may be closely related to the chronic complications of diabetes and are compatible with the knowledge of the literature.
