Predicting the development of the microangiopathy in TYPE 2 diabetes: Hemogram parameters relationship between microalbuminuria

Abstract (EN)
Introduction Diabetes mellitus (DM) is a chronic metabolic disease, which is common in the society and can cause serious physical disorders and death. It is estimated that 592 million people are expected to be diabetic in 2035. The global prevalence of DM is estimated to be %10.1. According to the latest studies, the prevalence of DM in our country is approximately two times of the 2007. Health care spending are increasing because of diabetes and its complications. Our aim in this study is to determine the relationship of MPV, RDW, PDW and NLR on microlbuminuria and glicemic control . By the help of the diabetic nepropathy at early steps. Materials and Methods This study is made single centerred retrospective definitive pattern . 979 patients with type 2 diabetes, who applied Adnan Menderes University Endocrine and Metabolism Clinic, were included in this study and 24-hour urinary albümin was studied in these patients. Because of the exclusion criteria, 268 patients were included in the study. Shapiro-Wilk test, Chi-square, Kruskal - Wallis and Mann-Whitney U, Spearmann and multiple logistic regression analysis was performed. Results 158 patients (%59.0) were male and 110 (%41.0) were female. The averege age of patients were 53.5 ± 11.5 (minimum age 26, maximum age 85; median: 54.0). The age range of 26 patients (%9,7) were between 18-39, 201 patients (%75,0) were between 40-64 and 41 patients (%15.3) were 65 years old or older. Men (51,0.11,1, minimum age 26, maximum age 77; median: 52.0) were younger than women (57.0 ± 11.1; minimum age 37, maximum age 85; median: 57.0 ) (p = 0.000). According to multiple logistic regression model positivity of microalbuminuria in 24 hour urinary test is 4.3 times more for the MPV values 10,25, 2,7 times more for the PDW values over 16,05, 2.4 times more for the NLR values over 1,96, 3.6 times more for the HbA1c values over 9,05. Sufficient level of significance (p=0,054) isn't shown either; HDL levels of less than 38.5 were detected in 2.4 times more microalbuminuria. According to the multiple logistic regression model for better glycemic control ,1.7 times better glycemic control was determine in the patients older than 54,5 years old and 2.5 times in triglyceride lower than 149,5. Results and Discussion The study showed a significant relationship between microalbuminuria and low MPV values; however both high and low MPV results are available in the literature. Significant association between microalbuminuria and high PDW values was seen in our study which is similar with the literature. A positive directional relationship is found between microalbuminuria and increase NLR which is similar as the literature. Keywords: Diabetes, Microalbuminuria, Hemogram Parameters
