Evaluation of the patients whose urine cultures have been taken at the emergency service because of urinary tract infection suspicion

Abstract (EN)

In this study we aimed to evaluate the patients?, who are considered as having a urinary tract infection at the emergency service, and whose urine culture has been sent, cultur results, antibiotic resistance and the parameters related to urinary tract infection. The study has been performed by retrospectively scanning the folders of the patients, who are over the age of 16, and whose urinary cultures have been taken, referred to The Emergency Service of Akdeniz University between the dates of 01 January 2010 and 31 December 2010. 444 patients, whose urinary cultures have been taken at The Emergency Service of Akdeniz University, have been included to the study. 42.3% of the patients were male. The most common pathogen reproduced in the urinary cultures was E.coli. Ciprofloxacin resistance determined as 26.9%, trimethoprim sulfamethoxazole resistance determined as 37.8% at the study. When the appropriateness of starting or not starting a treatment at the first reference has been compared it is determined that in 130 (29.3%) patient the appropriate treatment had been started. In logistic regression analysis, which is done to state the parameters related to urinary tract infection, nitrite positivity at strip test (odds ratio: 8.27 95% CI 3.84-17.80), leukocyte presence in urine (odds ratio:3.28 95% CI 1.73-6.23), and dysuria (odds ratio:2.25 95% CI:1.38-3.67) are found statically significant. According to our study results, it is determined that there is a high rate of resistance to the antibiotics which are used for urinary tract infection. When this situation cosidered, it might be appropriate to start antibiotic treatment, and send urine culture for the patients who are considered as having a complicated urinary tract infection.
