The effects of blood transfusion on lung function in open heart surgery

Abstract (EN)
Since open heart surgery is intended to the main source of body perfusion, it is inevitable to use blood and blood products in this surgical field. Even if there is no need for blood products, they are kept ready for immediate surgical complications. Another organ affected by open heart surgery is a general effect of anesthesia induction and as well as pulmonary complications due to the heart lung machine used in cardiopulmonary bypass. In our study, we examined the relationship between these three entities, namely open heart surgery, blood use and pulmonary function. For this purpose, the retrospective data of 106 patients with isolated coronary aortocoronary bypass graft surgery between 2015-2018 at Adiyaman University Education and Research Hospital Cardiovascular Surgery Clinic were included in the study. The patients were divided into three groups as patients without blood product used Group 1 (n = 25), only autologous blood used Group 2 (n = 29), and Group 3 (n = 52) with non-autologous blood product used. The obtained data were analyzed statistically preoperatively, intraoperatively and postoperatively in three groups too. When the patients were evaluated according to the preoperative demographic data, no statistically significant difference was found between the patient groups and the results were similar to the literature. In intraoperative data, the values that could extend the operation time were found to be significantly higher in Group 3. The values in the postoperative data were significant in terms of drainage, revision, atelectasis, Pa / FiO2 <300 statistics in Group 3 in parallel with intraoperative data. One of the reasons that negatively affects lung function after open heart surgery is the use of blood and blood products. Therefore, it should always be kept in mind to try to minimize the use of blood and blood products to prevent potential complications in this patient group.
