Assessment of ischemia modified albumin, total antioxidant status and total antioxidant capacity in diagnosis and follow up of celiac disease in children

In our study, we aimed to investigate oxidative stress markers which is an important factor in pathogenesis of celiac disease (CD) in children. For this purpose, we evaluated ischemia modified albumin (IMA), total oxidant status (TOS), total antioxidant capacity (TAC), sulphydryl (SH) and advanced oxidation protein products (AOPP) which are oxidant and antioxidant markers that had not been evaluated in CD before in the course of diagnosis and following gluten-free diet. A total of 37 patients who were diagnosed with CD at Pediatric Gastroenterology Clinic of Akdeniz University, School of Medicine between March 2011 and March 2013 and 29 healthy children were enrolled in this prospective study. While oxidant markers, IMA, TOS, AOPP levels were significantly higher in patient group compared to control group, antioxidant markers, TAC and SH levels were found significantly lower in patient group compared to control group. We also detected that dTG IgA showed a highly positive correlation (r=0.761, p=0.001) and IMA showed moderately positive correlation (r=0.647, p=0.001) with Marsh-Oberhuber histopathologic stage. We found that IMA, TOS, AOPP levels were significantly lower and TAC and SH levels were significantly higher after gluten free diet compared to pre-diet period . In conclusion, we detected that oxidative stress plays a role in pathogenesis of CD, this can be evaluated with oxidative stress markers and oxidative stress resolves after gluten-free diet. We also detected that IMA is a marker which shows a moderately positive correlation with histopathologic stage
