An investigation of the effect of peroperative blood usage on liver functions in open heart surgery patients 65 years and older with extra korporeal circulation technique

Along with the developing technologies and facilities, the number of elderly individuals in society and the number of patients referred to cardiac surgeons are increasing steadily. In this study, we investigated the effect of homologous blood usage on patients' liver function in open heart surgery with extra corporeal circulation supported by elderly patients aged 65 years and over according to World Health Organization. Our study was carried out in Dokuz Eylül University Hospital Cardiovascular Surgery Clinic based on retrospective file scan data of 185 patients who underwent open heart surgery with extra corporeal circulation system between January 2011 and December 2014. This study included patients who were 65 years of age or older, cross clamped on the aorta, and who did not exceed the pump duration of four hours. Patients, who had liver or renal failure, cardiac surgical history, exitus in the first postoperative 12 hours, chronic obstructive pulmonary disease, deep hypothermic circulatory arrest, alcohol or substance abuse and emergency cases were removed without study. A total of 185 patients, 74 females and 11 male, were included in the study. The patients were divided into two groups, one with blood (Group I) and one without blood (Group II). There were no significant differences between groups in terms of sex, age, and cross clamp time, but there was a significant difference between mean cardiopulmonary bypass durations. It was thought that this difference might be due to technical reasons or the number of patients between the two groups. The alanine aminotransferase and aspartate aminotransferase values, which are important predictors of liver function in patients, were compared between the two groups as pre- and postoperative first two days. There was a significant increase in the value of the group using blood compared to the group without blood. Finally, platelet counts of the two groups were compared before and after the first two days and a significant decrease in the values of the patients in group II was calculated The reason for our study is to monitor the effects of cardiopulmonary bypass and blood transfusion on liver function and guide our plans to reduce this complication in this direction. Our hypothesis was accepted that the use of homologous blood in operations performed with open heart surgery with extra corporeal circulation system as a result of our work has a significant effect on the liver in the postoperative early period.
