The results of salter osteotomy for thr treatment of developmental dysplasia of the hip

Abstract (EN)

There are couples of surgery techniques for the anatomical and functional treatment of developmental dysplasia of the hip. Robert Bob Salter has postulated that the main problem during the treatment of developmental dysplasia of the hip as the stability of reduction and he described the innominate osteotomy at 1961. In our study, we observed the clinical and radiological results of our patients who are treated with open reduction and Salter innominate osteotomy. We aimed to discuss the success of treatment and the factors which are affecting the prognosis. 27 hips of 20 patients out of 87 patients were included in our study who meet the criteria and willing participate between 1993-2012 who treated with Salter innominate osteotomy fort he developmental dysplasia of the hip. 17 female, 3 male patients were treated. The average age was 23.7 months (16-50 months) during the treatment. The mean duration of follow up was 72.18 months (22-128 months). The average asetabular index angle was 39.1° (32°-45°) during pre operative radiological evaluation. At the post operative evaluation the average asetabular index was 15.7° (10°-22°). McKay clinical evaluation criteria, we reached 18 hips excellent (66.7%), 9 hips good (33.3%) results. Also, 2 hips (7.4%) were excellent, 24 hips (88.9%) were good results, 1 hip (3.7%) fair result according to radiological evaluation system. Wiberg's CE angle average was 22° (15°-22°). Avascular necrosis was observed in 15 hips (55.5%) according to Kalamchi MacEwen avascular necrosis criteria, 13 hips (86.6%) were type 1, 2 hips (13.3%) were type 4. Our results were in accordance with the literature. One stage operative procedure consisting of open reduction and Salter's innominate osteotomy is preferable technique for the treatment of developmental dysplasia of the hip. Salter innominate osteotomy provides good covarege for the hip adviced for the patients who are treated under 4 years of age for good results
