Evaluation of two different total mesialization protocols with miniscrews support in agenesis congenital maxillary lateral by finite element analysis

Abstract (EN)

Aim: To examine the force and stress distributions in the dentoalveolar structures caused by two different total mesialization methods in congenital bilateral missing maxillary lateral teeth cases by using finite element analysis method. Material and Method: Two different models were applied by using finite element analysis. In both models, miniscrews were placed in the palatal region. In the first model, 450 gr force was applied to the miniscrews with chain elastic from the transpalatal arch. In the second model, the central teeth and miniscrews were combined with the appliance, an elastic chain force of 450 gr was applied to all teeth over the arch wire. Von Mises stress, maximum and minimum principal stresses, displacement values were obtained from the models and evaluated comparatively. Results: Due to the direct application of force to the palatal miniscrews used in the first model, high Von Mises stress was observed in the neck. Since the force is not directly applied to the miniscrews used in the second model, the Von Mises stress was not observed as much as in the first model. In the first model, the teeth with the most movement are the maxillary first molars, while in the second model, the canine teeth. In the second model, compression and tensile stress is observed in the maxillary anterior teeth with the effect of the elastic chain, while the tension in the maxillary anterior teeth is almost zero in the first model. While the amount of movement seen in the maxillary teeth in the transversal and sagittal planes is higher in the second model, it is seen less in the first model. Conclusion: It was determined that the highest stresses were in the miniscrews in the first model and in the maxillary anterior teeth in the second model. It was observed that the first molars were displaced the most in the first model and the canines in the second model.
