Neutrophil / lymphocyte ratio prognosis predictive value of colorectal cancer

Abstract (EN)
Purpose: In our study, we aimed at determining the tumor size of the neutrophil lymphocyte rate, positive lymph node rate, and the relation to the other prognostic factors like serum CEA and CA 19-9 levels in the cases with colorectal cancer which the resection was made. Material and Method: After local ethics council confirmation is had, the numeric statistics of patients who are applied resection in elective circumstances by the diagnosis of colon and rectum cancer by General Surgery Unit between 2008 and 2012 in the hospital of Adnan Menderes University Faculty of Medicine and the data relevant to 146 patients, were made using the information managing system of the hospital and examining the folder data retrospectively. Demographic data of the patients included in the study, their clinical follow-ups, laboratory parameters, the records of screening and pathology were examined and the essential data were included in the study. In this way, the relation between the rate of neutrophil lymphocyte and the other prognostic factors were researched. Findings: The 61% of the cases included in the study is male and 39% of them is female. The average age of the male cases is 66±13 and the female ones? is 64±12. The most frequent location place of tumor is rectum (36, 3%), secondly sigmoid colon (21, 9%) and transverse colon (2, 7%) at minimum level. In compliance with TNM staging system 69,9% of the cases have T3, 19,2 % of them have T2, and 11,0% of them have T4 stage tumor, and 30,8% of the cases of 58,2% which lympho node involvement was observed have N1 and 27,4% of them have N2 involvement. The average of serum CA 19-9 levels of the patients is 72,8±351,7 and serum CEA level average is 171,7±1699,2 and the average of neutrophil lymphocyte is 4,0±3,5 and median is 3,1. In our study a meaningful relation between NLO and the location of tumor (p=0,606), tumor size (p=0,657), positive lympho node rate (p=0,644) couldn?t be found statistically. In our study, while of the factors which affect the prognosis, increased tumor size and lympho node involvement is correlated with pre-operative serum CEA level, a relation between CEA level and NLO couldn?t be found. In our study while in the cases with colorectale cancer a relation between tumor stage out of the factors which affect prognosis and serum CA 19-9 levels and a relation between serum CA 19-9 level and NLO couldn?t be found, a moderate relation between serum CEA level and serum CA-19-9 level has been founded. Conclusion: A meaningful relation between neutrophil lymphocyte rate which is related to prognosis in patient who has various cancer types and the location of tumor, serum CEA level, tumor size and positive lympho node rate. Neutrophile lymphocyte rate is a simple analysis, a laboratory variable and the indicator of pro-tumor inflammatory balance state, the relation between an increased neutrophil lymphocyte rate and bad oncological way is still a matter of wonder. Contradictory results in close dated literatures are available. Whether hematological markers are much cheaper and fast laboratory parameters in comparison with the convectional markers like serum CEA, CA 19-9 and CA 72-4, immune response induced by tumor has a complicated structure and neutrophil lymphocyte rate response provides a look from only one aspect. Even if neutrophil lymphocyte rate isn?t evaluated as a prognostic factor only by itself, it can be useful in terms of the fact that it can provide additional point of view for clinician when evaluated with other factors. We have the idea that emphasizing that the necessity and the continuity of the studies relevant to this subject on which consensus hasn?t been provided are significant is useful.
