The investigation of interaction between ABCB1 genetic polymorphisms and Fentanyl effects

Abstract (EN)

It?s known that drug response among the individuals in a population as well as among the populations from different regions has varied in a wide range. This interindividual dissimilarity is probably also caused by the differences in the activity of enzymes responsible from drug metabolism. However, nowadays it?s clear that not only enzymes but also drug transporters and receptors have also genetic variations.There are studies demonstrating fentanyl?s being a substarte of P-gp, coded by multi drug resistance (ABCB1) gene.In this study, the association with ABCB1 C1236A and C3435T gene polymorphisms and the adverse effects of fentanyl on central nervous system were investigated. Blood samples were collected from 83 patients undergoing elective lower extremity surgery for assessment of ABCB1 C1236T and C3435T gene polymorphisms. During the operation, the patients were followed by BIS monitorization and OAA/S scale for sedation and respiratory rate and saturation for respiratory side effects. We found the frequencies of the alleles C 40%, T 60% for C1236T, and 56% and 44% for C and T alleles respectively for C3435T. Thus, the results were similar like in the other studies performed in Caucasians and other Turkish populations.In terms of sedation parameters and hemodynamic variables, the results were not statistically significant. However, wild individuals without T allel are prone to less susceptibility to respiratory depressive effects of fentanyl with C3435T polymorphism, while wild individuals with C1236T polymorphism are prone to more susceptibility to adverse effects of fentanyl.In conclusion, there was no association between ABCB1 C1236T and C3435T gene polimorphisms and adverse effects of fentanyl. Further studies with larger sample sizes are required.
