A randomized clinical trial of clomiphene citrate versus gonadotropin for ovarian hyperstimulation in intrauterine insemination cycles for unexplained and male subfertility

Abstract (EN)
In this prospective, randomized study; the impact of gonadotrophin and clomiphene citrate on the success of COH / IUI cycles in patients with unexplained infertility and mild male subfertility was evaluated. For this purpose, we compared the use of gonadotropin and clomifen citrate?s efficiency in 329 cycles that belongs to total of 219 couples. Per patient pregnancy rates were as follows; crude, clinical and live birth rates were significantly higher in gonadotropin group ( 47.7 %, 43.1 %, and 37 % , respectively ) as compared to clomiphene citrate group ( 28.2 %, 28.2 %, 20 %, respectively), ( p <0.05 ). Per cycle pregnacy rates were as follows; crude pregnancy rate and live birth rates were significantly higher in gonadotropin group ( 31.4 % and 24.3 %) as compared to clomiphene citrate group ( 20.6 % and 13.8 % ) ( p <0.05 ). However; clinical pregnancy rate per cycle was not different between groups ( 28.4 % vs 20 %, respectively ) ( p> 0.05 ). The total average rate of pregnancy loss was 26.7 %; and the difference between the groups with regard to abortion rate was not significant ( 33.3 % vs. 22.6 p > 0.05 ). The multiple pregnancy rate was 11.2 %.As a result; gonadotropins in COH / IUI cycles has favorable effects on pregnancy rates when compared to clomiphene citrate.
