Efficacy of high dose inhaled fluticasone compared with systemic corticosteroid in children with an acut asthma exacerbation of asthma

Abstract (EN)
Asthma exacerbations occur with cough, wheezing, and dispne. Asthma exacerbations are important for morbidity, mortality and burden of asthma. Adequate oxygenation, frequent use of aerolized ß2 adrenergic agonists, anticolinerjik drugs and systemic corticosteroids are the mainstay of therapy for moderate and severe diseases. However, there is concern about safety of repetitive courses of systemic corticosteroids. Studies about using about high dose inhaled corticosteroid treatment for acute asthma is interested recently. Inhaled corticosteroids are potentially efficacious for hospitalization compare to plasebo during asthma exacerbation.Children will be evaluated for four hours at one hour intervals by asthma score, forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) before and after treatment with 4000 ?g of inhaled flutikasone or 1 mg of oral methylprednisolone per kilogram of body weight.Primary outcome of this part of the study is hospital admissions and secondary outcomes are asthma score, forced expiratory volume in one second (FEV1), peak expiratory flow (PEF) and oxygen saturation. In this study we have evaluated 35 patiens for fluticazon group, 27 patiens methylprednizolon group.Compare to admission time and fourth hour of treatment and well being of patient there was improvement in asthma score, FEV1, PEF and oxygen saturation.Finally during asthma exacerbation compare to high dose inhaleted fluticasone and systemic methylprednisolon treatment there was no differevce with asthma score, FEV1, PEF, oxygen saturation.Key words: Asthma, Asthma exacerbation, Inhaled fluticasone, Systemic corticosteroid
