Comparison of the type ix collagen levels in the cases with diabetic and non-diabetic lomber disc herniation

Abstract (EN)

Today, anabolic and catabolic factors involved in pathogenesis of the disc degeneration are described by modern technological investigations. This study is the first study on the comparison of the charecteristic immunohistochemical staining of type IX collagen which is in the structure of lumbar disc related to type II diabetes. Longer duration of diabetes, ensuring nutritional and functional sustainability of disc tissue is difficult. As result of micro- and macro-angiopathic complications, the nutrition of the disc and other tissues are impaired. Accumulated toxic oxygen radicals and lactate cause the impairment of regeneration ability of chondrocytes and fibroblast-like cells. As a result of the binding of advanced glycation end products in diabetic discs to collagens with non-enzymatic ways, the mechanical properties deteriorated. In additon, excess lumbar compressive loads in these patients who are mainly obese is another degenerative condition.Discectomy materials from 30 diabetic and non-diabetic total 60 patients were included in our study. These materials have been prepared as paraffin blocks. Sections were obtained with thickness of 4 ? m from these blocks and stained with polyclonal rabbit anti-human collagen type IX immunohistochemical dye in Ventana instrument. The intensity of collagen type IX staining, immune reactivity staining ratio, total staining score were found. These materials were immunohistochemically stained with Rabbit polyclonal antihuman collagen type IX dye in Ventana instrument. Collagen type IX in the diabetic discectomy materials associated to disc degeneration in diabetic group were found lower than in non-diabetic group (p=0,001**< 0,01**).The statistical correlations between the total score of the staining and the level of HbA1c, diabetes duration, age and operation levels were analyzed. The regression test was statistically significant in diabetes duration and the low result in the total score of the staining (ß=?0,016, t=?2,610, p=0,015*<0,05; F=3,688, p=0,017*<0,05; R2=0,270). This decrease was associated with HbA1c levels.As a result, diabetes is a factor changing the amount of type IX collagen which contributes to resistance of the disc, hence diabetes facilitate formation of disc herniation. When the amount of type IX collagen in diabetic patient group was compared with diabetic group, it was found lower in diabetic group.
