Evaluation of the relationship of kinesophobia with anxiety, upper extremity disability and quality of life in patients with frozen shoulder

Abstract (EN)
Frozen shoulder (FS) is a painful shoulder disease in which glenohumeral joint movements are actively and passively restricted in all directions. If not treated, it can cause various mental illnesses and disability. Kinesiophobia; It is defined as a fear of physical activity resulting from excessive sensitivity and disturbing sensation due to painful or repeated injury. In this study, it was aimed to evaluate the relationship of kinesophobia with upper extremity disability, anxiety and quality of life in patients with FS. Applying to Fırat University Faculty of Medicine, Department of Physical Medicine and Rehabilitation between September 2020 and March 2021; As a result of clinical evaluation and radiological examinations, those between the ages of 18-75 and who agreed to participate in the study; 48 patients who were diagnosed with DO and met the inclusion and exclusion criteria were in the patient group; A total of 88 patients were included in the study, including 40 healthy subjects aged between 18 and 75 years in the control group, who did not have shoulder disease, who met the inclusion and exclusion criteria. The TAMPA scale was used to evaluate kinesophobia in patients and control groups. Beck Anxiety Scale to evaluate anxiety in patient group; The Disabilities of the Arm, Shoulder and Hand (DASH) scale to assess upper limb disability; Short Form (SF)-36 was used to evaluate the quality of life. When the patient group and the control group were compared, the TAMPA scale was found to be significant in the patient group. According to correlation analysis, TAMPA scale in the patient group; While it was positively correlated with Beck-Anxiety and DASH scales, SF36-Physical function was negatively correlated with emotional role difficulties and social functionality. In patients with FS, kinesiophobia is observed to be associated with low quality of life as well as increased anxiety and disability in the upper extremities. Structural disorders and psychological factors should also be considered among the reasons for prolonged movement limitation and delayed recovery. For this reason, when patients with FS are evaluated for treatment purposes, it may be beneficial to apply psychological support treatments as well as medical and physical therapy.
