SHORT HEAD AND SUMMARY THE EFFECT OF GLUTAMINE ON INTESTINAL ADAPTATION IN THE SHORT BOWEL SYNDROME OF RATS Dr. Ahmet Deniz UÇAR DEÜTF Genel Cerrahi AD. Inciraltı - İZMİR Short Bowel Syndrome is a phenomenon mainly characterized by malnutrition, diarrhea and weight loss after significant length of small intestinal resection with different indications. An adaptation process characterized by gross and microscopic changes starts in the remaining small intestinal tissue oon after resection. If this process fails to progress well enough to meet the demands of the organism, patients will become partially or even totally dependent to total parenteral nutritional support. If this happens, numbers of life threatening complications and increased costs will develop afterwards. For this reason, lots of medical and surgical investigations have been performing to augment and fasten the adaptation process and to avoid from the complications. As a primary energy source for enterocytes and because endogenous production during stressful condition is not enough tp meet the metabolic demand, glutamin is one of the most promising nutrient as a medical rhanagement of SBS and mostly studied one alone or in combination with other therapeutic agents. Glutamine is introduced as a nutritional component into the to the treatment of SBS especially after commercial formulations have become available, but beside of prior experimental and clinical trials sshowing the beneficial effects with the combination with other therapeutical agents, latter studies unfortunatelly have not shown the same effects. Parenteral glutamine administration in the SBS of rats were evaluated in this study to make a review of conflicting results on SBS treatment and anastomotic wound healing. Average weight of 187 g (171-205 g) 14 male Wistar albino rats were divided in to two groups. fter 75% of small bowel resection which is shown to be enough to create SBS, subcunateous lutamine and the same volume of 0,9% NaCI solution was given to the study and control groups espectively for 14 consequtive days. During the procedure, weight changes were recorded and at the nd of 14th day remnant small bowel tissues were examined for adaptation parameters which are ucosal thickness, crypt depth, goblet cell count, villus height. Newly synthesized collagen (acid- luable and salt-soluable fractions) were calculated as an indicator of wound healing at the i anastomotic site. Results have shown that parenteral glutamin does not enhance intestinal adaptation and wound healing in SBS. In another words, using parenteral glutamin in the treatment of SBS is not indicated and not cost effective.
