Abstract (EN)
THE EFFECT OF IL-1 GENE POLYMORPH SM ON GASTROESOPHAGEALREFLUX D SEASEDr. GÜLHAN AKÇ L DAYSALContrary to decrease in Helicobacter pylori prevalence and H. pylori related diseasesincidence, gastroesophageal reflux disease (GERD) and its complications are increasing bytime. The results of the studies examining the relationship between H. pylori and GERD, arecontradictory. One of the switching cytokines having effect on H. pylori related inflammation,is IL-1 β. In recent years it is shown that, proinflammatory IL-1 gene polymorphismincreases IL-1 β level and this is related to gastric atrophy, hypochlorhydria and gastriccancer risk. In our study, we aimed to investigate the incidence of this gene polymorphism inerosive and non-erosive GERD and if it has a preventive role against GERD.100 patients having gastroesophageal reflux symptoms were included in the study.Symptom scores were determined. Biopsies were taken for histopathology via endoscopy anddegrees of esophagitis were determined. 24 hours pHmeter were applied on patients and IL-1B-511/-31, IL-1 RN gene analysis were obtained.Among 100 patients who had endoscopy, esophagitis was found in 32 patientswhereas it was not found in 68 patients. There was not a statistically significant differencebetween erosive and non-erosive reflux groups for H. pylori positivity. IL-1 B-511 T/Tgenotype incidence was found significantly low in H. pylori positive and negative erosiveGERD groups. IL-1 B-31 C/C genotype incidence was found lower in erosive GERD groupbut the difference was not statistically significant.As a result we found that, IL-1 B-511 T/T genotype incidence in erosive reflux groupis significantly lower than non-erosive reflux group. Though most of our patients in erosivereflux group had mild erosion, our finding of significant difference made us think that, IL-1B-511 T/T genotype may really have an important role in erosion development in GERD.
