Abstract (EN)
Evaluation of umblical visceral sliding test by transvaginal ultrasonography probe in the prediction of intraabsominal adhesions in women who made laparoscopy with gynecological indications AIM: This study compares the results obtained by using the transvaginal ultrasonography (USG) probe as a superficial USG probe during the preoperative umbilical visceral sliding test with the intraoperative findings and evaluating the effectiveness of transvaginal USG in the detection of sub-umbilical adhesions. MATERIALS AND METHODS: This prospective study was conducted between January 2021 and January 2022 in Sakarya University Training and Research Hospital Gynecology and Obstetrics Clinic with 200 patients who had the inclusion criteria and would undergo laparoscopy for gynecological reasons. After the preoperative recording of 200 patients with TVUSG probe according to the presence or absence of a visceral sliding sign in the umbilicus, the presence of adhesions in the periumbilical area and the potential of TVUSG to detect adhesions were evaluated intraoperatively RESULTS: The sensitivity of TVUSG in the detection of adhesions in the umbilical area was 26.67% (12.28%, 45.89%), and the specificity was 92.62% (87.17%, 96.26%). Visceral glide test in the umbilical area detected adhesion with PPV in 42.11% (24.22%, 62.33%) and no adhesion with NPV in 86.25% (83.42%, 88.66%). CONCLUSION: It has been shown that the umbilical sliding sign evaluated with the transvaginal ultrasonography probe is an easy-to-apply, non-invasive, safe, reproducible new method that will reduce the complications that may occur during trocar insertion from the umbilicus during the operation and can be applied to all patients who have had previous surgery or are obese.
