Abstract (EN)

Purpose: To evaluate effectiveness and success of the whole body diffusion-weighted imaging (WBDWI) as a new method for the the detection of bone metastases which is more common in malignancies by comparing bone scintigraphy. Methods: Whole body diffusion MR (magnetic resonance) images of 21 patients with primary malignancies and bone metastases were retrospectively evaluated and the results were compared with bone scintigraphy findings. Results: İn 21 patients total 143 metastatic lesion detected by bone scintigraphy. Diffusion MRI could detect 96 of the 143 bone metastases. Out of 13 metastasis 12 cases could be detected with a percentage of %92.3. Vertebras are another success region in which 39 of 45 metastasis were monitored which corresponds to a percentage of %86.6. The detection ratio for the upper extremites, pelvis and cranium are respectively calculated as %73.6, %63.1 and %60. The zone which has the least success has been ribs. Only 16 cases out of 42 metastasis could be detected in ribs which corresponds to a %38.1. Spearman's correlation tests which is made between two methods, the findings revealed a high level of positive correlation in parallel to detection rate at the cranium, vertebra, upper limbs and lower limbs. Moderate positive correlation of findings in the pelvis. At ribs due to the low detection rate of correlation was not observed. Conclusion: According to findings we obtained in our study it is thought that the whole body diffusion MR imaging in the detection of bone metastases can be successful, especially in the axial skeleton. Failure rate in the ribs can be corrected by new technologies which develops magnetic susceptibility and fast sequences for increase to prevent movement artefacts through geometric resolution.
