Abstract (EN)
Adenoidectomy/Adenotonsillectomy is one of the most common surgical procedures in children. The main goal of adenoidectomy is to heal nasal airway obstructions and eliminate established pathogens that cause chronic rhinosinusitis, chronic otitis media with effusion, and upper respiratory tract infection. Curettage adenoidectomy (CA) has been widely adopted for over a century. Conventional curettage adenoidectomy is performed blindly through an adenoid curettage; this may be associated with the presence of residual adenoid tissue and increased postoperative morbidity, including Eustachian tube dysfunction, unintentional injury to peripheral tissue, and postoperative bleeding. An ideal adenoidectomy technique should be safe, fast, easy, and reduce postoperative complications and morbidity. Alternative adenoidectomy approaches such as microdebrider, electrocautery, bipolar coagulation, radiofrequency ablation and coblation techniques have been proposed with the developing technology recently. In this prospective descriptive study, from August 2022 to May 2023, 60 patients (a total of 120 ears) who underwent adenoidectomy or adenotonsillectomy due to adenoid and tonsil hypertrophy will be examined, with normal eardrum and normal tympanogram, in Harran University Training and Research Hospital, Ear Nose and Throat Clinic. The patients were randomly divided into two groups: 1st group, 30 people (n:30), those who had coblastor and adenoidectomy,2. Group 30 people (n:30) were determined as those who had adenoidectomy by curettage method, curettage adenoidectomy group and coblator device and adenoidectomy group, Tonybee and valsalva maneuvers will be performed tympanometry. Middle ear pressure changes on the 1st, 7th and 14th days before and after surgery. will be compared within and between groups. In addition, pain scale, duration of surgery and complications were also compared. Considering our results, adenoidectomy performed with endoscopy-guided coblator is performed with similar duration and safety compared to curettage adenoidectomy; It causes less postoperative pain by traumatizing the surrounding tissues and causes less dysfunction of the peripheral organs. In our study, unlike other studies, the traumatizing effect of ETF-1 test and Coblation on the Eustachian tube was less than that of Curettage adenoidectomy. and it was found to be superior to the classical method in terms of early complications. Keywords: adenoidectomy/ adenotonsillectomy, coblator, curettage, tympanometry.
