Abstract (EN)

Heart failure which is characterized with hemodynamic abnormality, disordered exercise capacity, neurohormonal activation and rapid progression is a complex syndrome that has high morbidity and mortality caused by cardiac mechanic failure at the systemic perfusion that is needed for tissues. The incidence increases with age. However, heart failure is clinically seen at terminal stages of cardiovasculer diseases. Advanced human life at last decades, drug developments in the treatment of cardiovasculer diseases, developments in invasive and non-invasive treatment options caused increased incidence of heart failure. Increased patient population leads to a lot of studies related to follow-up and treatment of heart failure. Despite all of these development, heart failure is a currently still high mortalite and morbidity course. Studies showed that 1 year and 5 years after the diagnosis of heart failure, 30-40 % of patients and 60-70 % of patients died due to primarily deterioration of heart failure or suddenly developing possible ventriculer malign arhythmia, respectively.Atherosclerosis is the most important cause of heart failure. Adipocytokines derived from adipose tissue is considered a participant of atherosclerosis. Vaspine (visceral adipose tissue-derived serpin) is a new and an important adipocytokine in regulation of glucose and lipid metabolism. Vaspin is a member of recently discovered serine protease family and is known that it is expressed at adipose tissue when insuline plasma concentration is reached peak levels in Otsuka Long-Evans Tokushima Fatty (OLETF) rats and obesity .In this study, we aimed to investigate that whether vaspin has a rol in the development and etiopathogenesis of chronic heart failure with comparison of vaspin levels between patients with chronic heart failure and healthy volunteers.In this study, total 80 cases consist of 40 patients with heart failure diagnosed as the echocardiograhic ejection fraction 40 % and/or below (n= 40) and control group (n=40) without heart failure symptoms and signs and preserved echocardiograrhically venticuler functions were enrolled between December 2009 ? May 2010 at Department of Cardiolgy, Faculty of Medicine, Firat University. Vaspin levels were studied with ELİSA at serum which were produced after blood centrifuge and stored at -20 0C.When compared with normal population, vaspin levels in patients with heart failure were found high ( serum vaspin levels; HF: 7.21±7.31, Control group: 4.67±4.94 ng/ml, p<0.05).As a result, serum vaspin levels in patients with heart failure were found significantly high when compared with control group. It is clear that randomized, prospective, long-term follow-up studies are mandotary for evaluation of predictive value of serum vaspin levels in patients with heart failure. We consider that our study is one of pioneer studies and ligh future studies in this area.Key words: Heart failure, vaspin, coronary artery disease.
