Effect of preoperative oral carbohydrate solution administiration on postoperative nausea and vomiting in patient undergoing laparoscopic cholecystectomy under Sevoflurane-Nitrousoxide anesthesia

Postoperative nausea and vomiting (PONV) is multifactorial and frequent problem and occured especially postoperative first two hour. Complications of drugs used to prevent PONV caused new resaorches. In our study, we aimed to research effects of preoperative oral carbohydrate solution administiration on postoperative nausea and vomiting in patient undergoing laparoscopic cholecystectomy under sevoflurane-nitrousoxide anesthesia.A total of 64 patients, both gender, normal body weightes, ASA risk status group I-II scheduled for laparoscopic cholecystectomy operation were included in this study. Patients divided into two groups randomly, Carbohydrate Group (Group CH) consumed carbohydrate solution evening before surgery 24 o?clock and 3 hour before surgery. In Control Group (Group C) after 24 o?clock traditional starvation administrated. In both group, for premedication midazolam 0.05 mg/kg i.m. used 30 minutes before operation. For anesthesia induction, 5-7 mg/kg sodium thiopental i.v. 2 µg/kg fentanyl and 0.1 mg/kg vecuronium used. Anesthesia maintained with %70 N2O and %30 O2 with %1-3 sevoflurane in both group. Preoperative, intraoperative (after induction, 5 minutes later entubation, intraoperative 15 minute sequentially), 5 minutes before extubation, 5 minutes after extubation and postoperative mean arteriyel pressure (MAP), hearth beat rate (Hbr), 5 minutes after extubation, 15, 30, 45 and 60 minutes, 2, 3, 4, 6, 8, 12 and 24. hours postoperative nausea and vomitting score (PONV score) and Visual analogue scale assessed.There no differance between age, gender, body weight, ASA, duration of anesthesia, MAB, HBR and total utilized analgesic amount between groups.Visual analoque scala values are lower in Group CH than Group C at 30 and 45 minutes (p<0.05) and 60. minutes (p<0.01); at 2. hour (p<0.05) Group C lower than Group CH.PONV recovery times 5 minute (p<0.05), 30 minute (p<0.001), 45 minute (p<0.01), 60 minute (p<0.05), postoperative 8 hour (p<0.01), were lower Group CH than Group C. Total analgesic consumption was lower Group CH than Group C (p<0. 001). Patients at Group CH were more satiafied than Group C (p<0.001).In conclution it?s determined that in carbohydrate solution administiration at laparoscopic cholecystectomy under sevoflurane-nitrousoxide group?s PONV ratio and antiemetic drug demand significantly reduced.Key words: Laparoscopic cholecystectomy, preoperative carbohydrate solution, postoperative nausea and vomiting.
