Abstract (EN)
Chronic Rhinosinusitis with Nasal Polyposis is a troublesome, chronic inflammatory disease that affects more than 10% of the adult population, causes decreased quality of life, lost productivity and lost time at work, and causes more than one million surgical interventions per year worldwide. As such, it is associated with high management costs and significant morbidity. It is a common heterogeneous disease. While nasal polyps are observed in a variety of clinical conditions, including cystic fibrosis and malignancy, they are more often associated with a subgroup of chronic rhinosinusitis, aptly termed chronic rhinosinusitis with nasal polyps (CRSwNP). Nasal polyps are benign and typically develop bilaterally in the sinonasal cavity. It is a middle-aged disease with a mean age of onset of 42 years and a typical age of diagnosis between 40-60 years. Most commonly, nasal polyps occur as bilateral inflammatory lesions originating from the ethmoid sinuses and protruding into the nasal airway below the middle turbinate. Nasal Polysopia is more common in men than in women. Chronic Rhinosinusitis with Nasal Polyposis is defined by both symptoms of mucopurulent drainage, nasal obstruction, facial pain, and decreased sense of smell lasting longer than 12 weeks, with corresponding radiological or endoscopic imaging of sinus inflammation and polyp tissues. The nose, paranasal sinuses and associated lymphoid tissues play important roles in homeostasis and immunity, and Chronic Rhinosinusitis with Nasal Polyposis significantly impairs these normal functions. The pathogenic mechanisms of Chronic Rhinosinusitis with Nasal Polyps have recently become the focus of intense research around the world and significant progress has been made. It is now recognized that the two main long-recognized forms of Chronic Rhinosinusitis, with and without nasal polyps, are heterogeneous based on the underlying mechanism, geographic location, and race. Loss of the immune barrier is a common feature of many forms of Chronic Rhinosinusitis, including increased permeability of the mucosal epithelium and decreased production of important antimicrobial agents and responses. In this study, the amino acid sequence in the pathological tissue and blood of patients with Nasal Polyposis and Chronic Rhinosinusitis, who applied to our polyclinic and was hospitalized in our clinic, where surgery was planned, and operated for diagnosis and treatment, will be examined. This study will be conducted on 30 patients who were diagnosed with Chronic Rhinosinusitis with Nasal Polyposis and applied to the Harran University Training and Research Hospital Ear Nose and Throat clinic with complaints of Chronic Rhinosinusitis between May 2021 and December 2021. Nasal endoscopic examination, paranasal sinus computed tomography and blood tests will be performed before the surgery. Tissue and blood samples taken will be kept at -80 C until the study is done. Tissue samples taken will be homogenized. Amino acid sequences will be examined in homogenized tissue samples and blood samples. The aim of our study is to determine the amino acids that may play a role in the early diagnosis and treatment of inflammatory processes before the onset of the disease in patients with Chronic Rhinosinusitis with Nasal Polyps, which are thought to arise as a result of inflammatory processes and triggers, and this research will lead to future research.
