Clinical and perceptional evaluation of crown lengthening procedure with piezo-assisted-surgery compared to conventional bone resection technique

The aim of this clinical study is to examine the Crown Lengthening Procedure (CLP) performed with piezosurgery in comparison with classical bone surgery in terms of clinical improvement and patient perception/comfort. CLP procedures are one of the most frequently applied surgical procedures in periodontology practice to obtain adequate crown length for different multidisciplinary purposes. Piezosurgery is an alternative method to traditional methods used in resective bone surgery, which is claimed to accelerate postoperative wound healing and increase patient comfort. Total of 24 teeth including incisors, canines and premolars with CLP indication for restorative / prosthetic purposes; randomly divided into piezosurgery (test, n:12) and conventional bone resection (control, n:12) groups. Color, presence of edema and aesthetic appearance parameters were evaluated on days 7, 21, and 42. Pain levels recorded by the patient using the Visual Analogue Scale (VAS) throughout the 1st week and on 21st and 42nd days postoperatively. As a result of the study, complications such as postoperative bleeding and hematoma were not observed in either groups. Redness and edema were observed significantly higher in the control group on the 7th and 21st days compared to the test group (p<0.05). Postoperative pain level was significantly lower in the test group compared to the control on the 6th day (p<0.05). The findings of the study support the idea that CLP performed with piezosurgery produces less pain and better postoperative recovery compared to the conventional technique. There is a need for studies in which more patients are included and recovery markers are examined to evaluate the efficacy of piezosurgery.
