Abstract (EN)

Aim: This study investigates the detection of celiac disease(CD) in patients diagnosed with autoimmune liver disease (AILD) accompanied by CD and its impact on clinics and treatment responses in AILD. Material and Methods: Between 2000 and 2023, 126 patients diagnosed with AILD were identified in the Gastroenterology outpatient clinic of Istanbul University-Cerrahpasa, Cerrahpasa Faculty of Medicine. The patients' data were retrospectively reviewed. 32 patients were excluded from the study due to insufficient data, and 94 patients were included in the study. Results: Extrahepatic autoimmun disease(EHAD) were found in %33,5(34/94) patients. The frequency of CD was 7.5%(3/40) in the autoimmune hepatitis(AIH), 2.7%(1/37) in the primary biliary cholangitis(PBC), and 8.3%(1/12) in the AIH-PBC overlap syndrome(OS). Among patients screened for CD, 55% of the AIH group were on immunosuppressive (ISx), while 27,5% were on steroids. In the AIH-PBC group, 58,4% were on ISx, and 8,3% were on steroids. Conclusion: Our study confirmed the increased frequency of CD in patients with AILD, and the presence of CD does not affect the treatment of AILD. In the AIH group, the frequency of CD is 7.5%, and it is similar in OS as well. It was observed that many patients with AIH and AIH-PBC received steroids and ISx during CD screening. Considering that steroids and ISx may affect anti-tTg antibody levels and duodenal biopsy results, screening for CD before initiating treatment in these patients would be more appropriate.
