The prevalence of temporomandibular joint disorders in hashimoto's thyroiditis, hyperthyroid and hypothyroid patients and their relationship with psychological status, quality of life and sleep quality

Abstract (EN)

The aim of this study was to determine the prevalence of TMDs in patients with Hashimoto's thyroiditis, hyperthyroidism, hypothyroidism; and to evaluate their relationship with sleep disturbances, oral health-related life quality and psychological states. This case-control study included 20 women who were diagnosed with Hashimoto's thyroiditis, 20 women with hyperthyroidism, 20 women with hypothyroidism according to their serum concentrations of TSH, fT4, fT3 and TPO-Ab and ultrasonography results; and 20 healthy women who were not diagnosed with a thyroid disease. Participants were examined by an endocrinologist in order to diagnose thyroid diseases and a dentist to diagnose TMD. For accurate diagnosis of TMD, Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) was used. Pittsburgh Sleep Quality Index (PSQI) was used to determine sleep quality, Oral Health Impact Profile (OHIP-14) was used to evaluate the oral health-related quality of life and Revised Symptom Checklist (SCL-90-R) was used to assess the psychological status. TMD prevalence was statistically higher in Hashimoto's thyroiditis group (85%9, when compared with hypothyroidism (35%) and control group (55%). There was no significant difference in the rate of good sleepers between Hashimoto's thyroiditis (50%), hypothyroidism (40%), hyperthyroidism (55%) and control groups (35%). In the hypothyroidism group, patients showed significantly higher values in the average psychoticism scores when compared with the control group. There was no significant difference between groups; Hashimoto's thyroiditis (41.2%), hypothyroidism (42.9%), hyperthyroidism (53.8%) and control group (36.4%) in terms of the relation between good sleepers and TMD. In conclusion, no significant relation was found between thyroid diseases and TMD. Hashimoto's thyroiditis, hypothyroidism, hyperthyroidism and TMD all have negative impacts on both sleep quality and oral health-related quality of life.
