Advantages and disadvantages of preservation and structural rhinoplasty

Aim: The aim of this study was to contribute to the literature by comparing the advantages and disadvantages of preservation and structural rhinoplasty and to make it easier to decide which surgical method is appropriate for patients. Material and Method: Rhinoplasty cases were retrospectively evaluated in Cerrahpaşa Medical Faculty-Otorhinolaryngology between 2019-2024. 50 preservation(PR), 50 structural rhinoplasty(SR) surgical measurements and postoperative first-year controls were included in the study. Results: The width between the preoperative-postoperative typing points, width of the dorsum at the rhinion and nasion, location and height of the radix, length of the nose, projection, alarrim-columella distance, type and alar lobule,base width, alarbase resection, the presence of saddle deformity, hump recurrence and dorsal widening in the supratip in the first postoperative year were compared between the two groups. In most of the compared measurements, results consistent with the literature were obtained. Among the parameters we compared, we predicted that alarbase enlargement and the need for alarbase resection may increase especially in PR because the dorsum is preserved and lowered down. We noted that alarbase resection was performed in 3 of 50 patients who underwent PR and 1 of 50 patient who underwent SR. Conclusion: PR and SR have advantages-disadvantages against each other and in our study, we especially focused on alar base resection among the parameters we compared. We didn't find a similar study in the literature. We didn't find a significant difference between the two groups in terms of alarbase resection.
