Prognostic factors in primary mucinous ovarian cancer

Abstract (EN)

Aim: This study aimed to examine the factors affecting overall survival and disease-free survival and perioperative morbidities taking into account histological subtypes in patients diagnosed with primary mucinous ovarian cancer in our institution. Materials and Methods: 61 patients diagnosed with primary mucinous ovarian cancer between 2007 and 2023 at Başkent University Ankara Hospital, Department of Gynecological Oncology were evaluated retrospectively. All specimens were reviewed by the gynecological pathologist. The diagnoses were validated according to the 2020 WHO classification and they were divided according to histological subtypes. Clinical and pathological features of the patients and histopathological features of the tumor were examined. 5-year disease-free and overall survivals and prognostic factors affecting them were evaluated statistically. Statistical analyzes were performed using the statistical software package SPSS version 25.0 (SPSS, Inc. Chicago, IL). Result: The median age at diagnosis of the patients is 45 (13-84). The median total follow-up period of the patients is 55 (2-190) months. The 5-year overall survival and disease-free survival of all patients were found to be 73% and 70%, respectively. In multivariate analysis of overall survival, it was seen that early/advanced stage (HR, 3.925; 95% CI, [1.199-12.845]; p=0.024) and histological subtype (HR, 11.499; 95% CI, [2.367-55.851]; p=0.002) affected overall survival. In the multivariate analysis of disease-free survival, early/advanced stage (HR, 6.467; 95% CI, [2.093-19.982]; p=0.001) and histological subtype (HR, 8.260; 95% CI, [1.684-40.512]; p=0.009) were associated with disease-free survival. It was determined that morbidity and perioperative complications did not affect overall survival and disease-free survival. Conclusion: It has been observed that the histological subtype affects the prognosis independently in primary mucinous ovarian cancer. The 5-year OS and PFS values of the expansile group are 94% and 91%, respectively. The 5-year OS and PFS values of the infiltrative group are 37% and 38% (p:0.000).
