Abstract (EN)

Cardiac remodeling is a major factor in determining the clinical process of the heart failure regardless of etiology. Matrix metalloproteinase, play an important role on ventricular remodeling. It is showed that matrix metalloproteinase levels in the blood are associated with progression of heart failure. Propeptide procollagen type-1 is serum biomarker of collagen biosynthesis. There is a positive correlation between the serum levels of propeptide of collagen type-1 and the volume of fibrous tissue identified in cardiac biopsy. The primary purpose of this study is before and after surgical correction of stenosis in congenital cardiac defects associated with right ventricular outflow tract , to evaluate the usability of procollagen type-1, matrix metalloproteinase-2 and matrix metalloproteinase-9 on prediction of the level of prognosis and remodeling of the patients. In this study 40 patients that operated due to the congenital heart disease with the help of cardiopulmonary bypass between February 2011 and January 2013. The research group of patients was created with right ventricular outflow tract obstruction. The control group of patients was created with no right ventricular outflow tract obstruction. On these patients, 24 hours prior to the surgery, and postoperative 24 ? 48 hours and 6th month arterial blood samples are taken and studied procollagen type-1, matrix metalloproteinase-2 and matrix metalloproteinase-9. The result are compared between the group intervened because of right ventricular outflow tract stenosis and the group without right ventricular outflow tract stenosis. It was significant that the high level of matrix metalloproteinase-2 and matrix metalloproteinase-9 in the early and mid-term following surgery. This height may be and indicator of myocardia remodeling. The high levels of procollagen type-1 in the patients followed and taken into surgery because of pulmonary stenosis constitute strong evidence about that these patients may have higher volumes of fibrous tissue
