Abstract (EN)
Spinal hypotension during cesarean section has been shown to reduce brain oxygen saturation by NIRS. However, monitoring of changes in brain oxygen saturation simultaneously with spinal anesthesia during spinal anesthesia and determination of effects on mother and baby is an issue that has not been investigated before. This will contribute to the literature. Near infrared spectroscopy (NIRS) is a non-invasive spectroscopic method that measures frontal lobe microvascular oxygenation using the infrared portion of the electromagnetic spectrum (800–2500nm). The cerebral oxygen saturation change of the patients included in the study was measured with a noninvasive method, with the help of probes (two) placed on the forehead of the patients. The placental oxygen saturation change was measured non-invasively by the obstetrician and gynecologist with the help of probes placed in the lower abdomen (approximately over the placenta). All probes were placed preoperatively. Measurements were made with placental oxygen saturation probes for 15 minutes and removed before the operation started. Changes in blood pressure and cerebral oxygen saturation were continuously monitored in the peroperative period. In our study; The aim of this study is to evaluate the effect of spinal anesthesia method on cerebral and placental oxygenation by using NIRS method in pregnant women who are planned for elective cesarean section. Placental oxygen saturation values were followed preoperatively, cerebral oxygen saturation values were followed during the preoperative and intraoperative period. In our study, a statistically significant correlation was found between hypotension due to spinal anesthesia and the decrease in placental rSO2 and cerebral rSO2 values. In conclusion, it is clear that the NIRS technique is effective and beneficial in terms of cerebral oxygen monitoring in preoperative and intraoperative periods in patients during pregnancy where physiological changes are very high. There is a need for more studies with more detailed measurements in terms of both pregnant and infant safety.
