Abstract (EN)

"Retrospective Investigation of Postoperative Pregnancy Outcomes, Length of Hospitalization, and Intraoperative Complication Rate in Cases Undergoing Uterine Septum Resection for Recurrent Pregnancy Loss, Primary and Secondary Infertility" Patients who underwent hysteroscopic uterine septum resection for recurrent pregnancy loss, primary and secondary infertility were retrospectively analyzed. We aimed to investigate the effects of septum resection on conception and pregnancy outcomes by evaluating preoperative and postoperative pregnancy outcome data. Between January 2016 and March 2023, 125 patients who were diagnosed with uterine septum and underwent hysteroscopy at Süleyman Demirel University Faculty of Medicine, Department of Obstetrics and Gynecology were analyzed. Patients whose information could not be reached and who did not meet our criteria were excluded from the study and a total of 92 patients were studied. Patients requiring hysteroscopy, laparoscopy-guided hysteroscopy and revision procedures were identified. Demographic information, preoperative and postoperative pregnancy data, intraoperative complications, amount of bleeding, and duration of hospitalization were processed as data forms. In our study, 92 patients who met our criteria were evaluated in our clinic. There were 29 (31.5%) primary infertile patients, 56 (60.9%) secondary infertile patients and 7 (7.6%) patients with a history of habitual abortion. When we analyzed the demographic characteristics of all groups, the mean total age was 28.64±5.81 years. Of the 92 patients, 90 (97.8%) had incomplete septum and 2 (2.2%) had complete septum. 78 (84.8%) of 92 patients underwent diagnostic laparoscopy and 14 (15.2%) underwent only hysteroscopic procedure. 7 (7.6%) of 92 patients underwent a 2nd session operation. The pregnancy rate was 68.4% preoperatively (63 patients), 66.3% postoperatively and remained unchanged, the live birth rate increased from 27.2% to 50%, the abortion rate decreased from 59.8% to 16.3%, and the complication rate was low (3.3%). An increase was observed in the average birth week. Hysteroscopic septum incision is a frequently preferred method today because it is easy to perform and has a low morbidity rate. We found that pregnancy occurred in many patients in the 12 months we observed, the live pregnancy rate increased and the number of miscarriages decreased. In order to see the effect of uterine septum incision alone, more comparative prospective randomized controlled studies including the untreated observation group and the septum incision group are needed.
