Angiotensin 2 Type 1/Type 2 receptor gene polymorphism distribution in patients with vesicoureteral reflux and recurrent urinary tract infection

Abstract (EN)

Urine escape back to the ureters and kidneys during voiding, is called vesico-ureteric reflux (VUR), depending on the insufficient valvular mechanism around the ureterovesical junction, for the anatomical and functional reasons. Recent studies focus on genetic factors in the development of renal scarring and reflux nephropathy (RN). The best known studies are related to the renin-angiotensin system. It has been reported that Tip1/Tip2 Angiotensin2 receptor gene polymorphisms (AT1R/AT2R) play a role in renal parenchymal injury. The purpose of our study is to investigate the role of AT1R/AT2R gene polymorphisms, in the patients VUR. Patients with the diagnosis of R.UTI renal scarring (+) 25 / renal scarring (-) 25, diagnosis with VUR renal scarring (+) 25 / renal scarring (-) 25 patients and 50 healthy children were included in the study, consulting in Pediatric Nephrology MD outpatient clinic. Including UTI group, 23 patients were male and 27 were female, with mean age of 6,9±0,3. 22 male and 28 female patients with VUR were evaluated, with a mean age of 7,3±2,9. Neurogenic bladder, lower urinary tract obstruction, patients with secondary reflux such as double collecting system and ectopic kidney and additional urinary tract malformations were excluded. AC genotype and CC genotype had a significantly higher risk of İYE when compared to the control group (p<0,05). AC genotype among IYE scar (-) patients. CC genotype among IYE patients scar (+) were significantly higher when compared to the control group (p<0,05). C allel percantage is higher compared to control group AC genotype had a significantly higher risk of VUR scar (-) when compared to the control group (p<0,05). While AT2R gene polymorphism was evaluated, GG genotype in R.UTI group is 3.2 times higher than the control group and that is found statistically significant G allel percantage is higher compared to control group (p<0,05).In our study, significance was not found for the AT1R/AT2R gene polymorphism with development of scar in VUR, also it was determined that, it plays a role in the development of renal scarring regardless of reflux at R.UTI. This situation suggests that different genetic factors may play a role in the development of scars in VUR, together with, which lasts for a preliminary study, that should be supported by a greater number of patient population.
