Abstract (EN)
Early enteral immunonutrition makes differences in stress response that depend on cytokines, make an optimal inflammatory and immune response at dynamic process of SIRS through sepsis, effective in increasing survival.In our study, we research effect of standard and contained glutamine products on early and late nutrition regimen in inflammatory response at critical patients.Between 30-60 years old, 40 patients that need ventilation support, with randomisation divided into 4 groups. Group SE: standard early enteral, group SG standard late enteral, IE: early enteral umminonutrition, IG determined as late enteral immunonutrition. In early enteral nutrition group, nutrition started within first 24 hours of treatment, late groups nutrition started after 72 hours. Prealbumin, albumin CRP, IL-6, IL-10, APACHE-II scores and mortality were calculated for 1. 5. and 7. days of admission.There was no difference in demographic values, BMI and daily calories intake between groups. Group SE decreases detected at prealbumin levels (p<0,05). For albumin levels there were no difference between groups. Although there was decrease detected in group IE and group IG than group SG, there was no difference in SE. In SE there was increase at 5. and 7. days (p<0,05).In SG, there were increases in IL-6 levels at 5. and 7. days than SE. In IE and IG there was decrease detected than SG at 5. day. In SG group there were decrease in IL-6 levels at 7. day than SE. In IG, there was decrease detected than SG, and increased than SG.In IG there were increase detected at IL-10 levels at 5. and 7. days. In IG, there were increases detected at APACHE-II detected than SG at 5. day. There was no difference detected for mortality.It?s concluded that glutamine rich early enteral immunonutrition assessment efficiently suppress systemic inflammatory response, without effecting to mortality and biochemical parameters.Key words: Glutamine, early enteral nutrition, immunonutrition.
