Comparison of Troponin T (TnT) and High Sensitive Troponin T (hsTnT) value which patients apply the emergency service and suspected acute coronary syndrome

Coronary heart disease is the leading cause of death in industrialized countries. AMI (Acute Myocardial Infarction) is diagnosed with the patient?s clinical status, laboratory markers, and electrocardiography (ECG) findings. The shorter the time from arrival of the patient to the clinic after the start of the chest pain through to the diagnosis, the greater the chance of starting reperfusion therapies before the necrosis occurs. It is emphasized that cardiac enzyme changes are more important than ECG changes in early diagnosis of acute coronary syndromes.The purposes of this study were to ensure early initiation of treatment with early diagnosis of patients, to prevent emergency service overcrowding by shortening the duration of follow-up, to reduce costs, and to increase patient satisfaction and theraphy.This study was a prospective observational clinical study. Forty five patients who fulfilled the inclusion criteria were enrolled. TIMI risk score was applied to patients and patients with a TIMI score of 3 or more were included in the study. Serum samples were obtained at admission time and, 3rd, 6th, and 12th hours of the application. SPSS 16.0 was used for all statistical analysis. Mann Whitney U test, Spearman?s correlation analysis, odds ratio, and Pearson chi-square test were used for data analysis.Results were evaluated in 95% confidence interval, significance (OR) p<0.05 and as a two-way. Seven (63.6%) of 11 patients diagnosed with nonspecific chest pain had negative hsTnT but 4 (36.4%) patients had positive hsTnT. All of these 11 patients (100.0%) had negative TnT. Nineteen (61.3%) of 31 patients diagnosed with unstable angına pectoris (USAP) had negative hsTnT whereas 12 (38.7%) patients had positive hsTnT test. All of these 31 patients (100.0%) had negative TnT. Previous studies show that where TnT values were detected negative, hsTnT values were positive in same cases. In our study it has observed that TnT was insensitive for some values which were sensitive for hsTnT (p<0.05). Upper reference limit for hsTnT test was 99th percentile. Coefficient variation (CV) was lower than 10%. This level of testing was the highest currently available level.In conclusion, in early diagnosis of acute coronary syndrome hsTnT may be safely used.
