Assess the reliability of patient history and pregnancy prediction in the exclusion of pregnancy and early complications of pregnancy detection with the evaluations of serum beta-hCG levels in the emergency department

All women in the reproductive age is assumed to be pregnant until proven otherwise, and pregnancy test is used very often intended to exclude pregnancy. In our study we aimed to assess the reliability of pregnancy who patient history and prediction of pregnancy.The study was performed prospective and a descriptive survey. All female patients presenting the emergency department with any complaint, studied the serum beta-hCG level, between the ages of 18-50, meeting the study criteria were included into the study. Electrochemiluminescence immunoassay (ECLIA) method was used for the measurement of serum hCG level. Pearson ?2 and Fisher?s exact test were used to compare the qualitative data. Statistically, p <0.05 was considered significant.During the study in three months period, our emergency department admitted 2415 female patients whose age ranges are from 18 to 50. Serum beta-hCG test was required from 965 ones of these patients. The study included 470 patients and 15 (3.2%) pregnancies were detected. The mean age of the patients was 29.2 ± 9:18 (mean ± SD) and the most frequent ages of the applicants are between 18-25 (46.4%). Looking at the demographic characteristics of the patients; 31.5 % were housewives, 30.2% were students, 50.4% were married, 46.6 % had a university education, present in the patient population. The most common complaints were abdominal pain (30.2 %) and nausea-vomiting (22.3 %). Housewife (Pearson ?2 test, p= 0.045), married (Pearson ?2 test, p=0.010) and admitted with complaints of vaginal bleeding (Fisher's exact test p < 001), amenorrhea (Fisher's exact test p <001), heartburn (Fisher's exact test p=0.003), bloating (Fisher's exact test p=0.002), nausea and vomiting (Fisher's exact test p=0.002), statistically significant correlation was found compared to other groups in detection of pregnancy. Menstrual abnormalities by date of last menstrual period to plan the patient's pregnancy, the presence of sexually active made meaningful detection of pregnancy. Prediction of the patient ``I'm not pregnant?? and prediction of physician ``low probability of pregnancy?? were found significant to rule out pregnancy.We think that serum hCG test is unnecessary for the patients presented to the emergency department with the complaints other than abdominal pain and vaginal bleeding, menstruation timely and normal according to the last menstrual period, who had normal mental status, ``definitely I'm not pregnant?? saying patients.Key words: Emergency department, reproductive female patient, the patient's prediction of pregnancy, serum beta-hCG test
