Abstract (EN)

Investigation the persistence of H.pylori in the pediatric population underwent adenotonsillectomy with PCR at adenotonsiller tissue and with ELİSA procedure at serum and comparing of results with urea breath test. H. pylori (Helicobacter pylori) is a gram negative, helical, microaerophilic, 0,5-0,9x3 µm in size, motile bacteria that colonized within the gastric mucosa. H. pylori commonly found all over the world may colonize in all ages from childhood. It can be found much more in developing and undeveloped countries than developed countries. It can not be revealed exactly how the contamination is occurred. Possible contamination ways are accepted as fecal-oral, oral-oral, gastro-oral and iatrogenic. Reviewing the literature, in extensive researches, relationship between h.pylori and gastritis, peptic ulcer, peptic carcinoma and MALT (mucosa-associated lymphoid tissue) lymphoma was shown. The relationship between non-gastrointestinal disorders such as GERD, Atherosclerosis, ITP, iron-deficiency anemia and exstragastric colonization is still a controversial issue that continues to be investigated. We aimed to determine the prevalence of H. pylori in adenotonsillar tissue by combining different methods of diagnosis with this study. We tried to reveal possible extragastric colonization in adenotonsillar tissue in patients underwent adenoidectomy, tonsillectomy and adenotonsillectomy by PCR and serological tests with serum. Urea breath test which is an easy and non-invasive method is applied to detect possible postoperative gastric colonization changes. Our research was carried out prospectively between Jan. 2011 and Aug.2015 in Inonu University Otorhinolaryngology department, Clinic Microbiology department and Pediatric Gastroenterology department. The patients applied us with snooring, open mouth sleeping, chronic or recurrent tonsillitis, sleep distubances were admitted to our study. One hundred twenty four patients were included in our study. Ages were between 3-18 (mean 8,4), 57 (% 46) of 124 patients were female and 67 (%54) were male. Ninety one patients were underwent adenoidectomy, 6 of them had tonsillectomy and 25 patients had adenotonsillectomy. One hundred eighteen adenoids and 63 tonsils materials were investigated by PCR for the presence of Hp. In the PCR analysis, 9 (14,2%) of 63 tonsils and 25 (21,1%) of 118 adenoid tissue were found positive with Hp DNA. Thirty four from a total of 181 (18,7% ) adenoid and tonsillar tissue were found positive with Hp DNA. In our study 46 patients were aplied with Urea Breath Test. Hp DNA was found positive in 18 (%39,1) patients from a total of 46 patients. Five (% 27,8) from 18 Hp DNA positive patients and 11 (%39,3) from Hp DNA negative patients had positive Urea Breath Test result. However there was proportionally difference in the group with negative PCR, In PCR analysis, there was no statistically significant difference in terms of UNT positivity between the helicobacter positive group with negative group. In conclusion, in our region, which is endemic to the H. pylori, We found the prevalence of H. pylori as %18.7 in adenotonsillar tissue. We believe H. pylori can be found in adenotonsillar tissue as an extragastric reservoir. This reservoir may play a role in the transition oral-oral and orogastric way. However, we found that removing of adenotonsillar tissue which is an extragastric reservoirs had any effect on colonization of stomach. Our results should be supported by further studies in order to develop. Key words: helicobacter pylori, adenotonsillar tissue, ekstragastric rezervoir, urea breath test.
