Investigation of genotype, mutation and antiviral drug resistance of HBV and HCV agents detected in Afyon region

Abstract (EN)

This study was performed between September 2007-March 2008 on 74 patients who admitted to Afyon Kocatepe University Hospital policlinics and Blood Bank or indivudials who were detected hepatitis during screening. These were chosen from 44 HBV infected (25 of them never treated with LAM), 30 HCV infected patients. The genotyping, mutational analysis and antiviral drug resistance examinations were performed on 44 serum samples with positive HBV DNA. The region between 80-260 codons in HBV DNA polymerase gene at Hepatitis B, was amplified by PCR and then sequence analysis was performed. It was determined that all the patients were belonging to genotype D. The mutational analysis and antiviral drug resistance tests which were done at the same sequences; 6 patient (13.6%) M204I mutation (LAM resistance), 2 patient (4.5%) Q215S mutation (LAM resistance), in one each patient (2.3%) V214A mutation (ADV resistance), L80I+M204I mutation (LAM resistance), A181T+N236T mutation (ADV resistance), V84M+V173L mutation (ADV and LAM resistance), L80V+M204I mutation (LAM resistance), Q215S+M204I mutation (LAM resistance), and M204I+L180M mutation (LAM resistance) were detected. The mutational analysis and antiviral drug resistance was not showed in the rest of 29(65.9%) patients. The genotyping (ABI 310, AB) was performed by sequencing analysis on 30 serum samples with positive HCV RNA. The sequences which determined genotype in 5'NCR region of HCV genome were analysed for genotyping. It was found on 19 patients (63.3%) subtype 1b, 6 patients (20%) subtype 1a, 4 patients (13.3%) subtype 4a and 1 patient (3.3%) subtype 1c of 30 patients, who was HCV RNA positive. Genotype 1 was determined predominantly in total of 26 (86.6%) patients. In conclusion, genotype 1b was detected in most of the HCV patients, genotype D was detected in all of HBV patients. It is considered that, detected genotype, antiviral drug resistance and mutations will guide us for clinical management and treatment of chronic hepatitis patients.
