Skin manifestations in dialysed patients with chronic renal failure at area of Afyonkarahisar

Abstract (EN)

A number of skin diseases can be seen in chronic renal failure. Theirincidence changes between %50?100 in different series. Skin manifestations canbe nonspecific like half and half nail, change of pigmentation and xerosis or bespecific as uremic pruritus, acquired perforating dermatosis, pseudoporfiriacutanea tarda, calcipylaxis and nephrogenic fibrosing dermopathy. The toxicmaterials which can not be eliminated from the body, drugs which are used by thepatients and the disordered cellular immunity are the main causes of these skinmanifestations.The purpose of this study was to evaluate the prevalence of dermatologicalproblems among hemodialysis patients with chronic renal failure.In our study; diabetes, hypertension and glomerulonephritis are the mostcommon causes of chronic renal failure which were likely evaluated in withprevious studies. At least one of the many skin diseases has been detected in allpatients. Kserosis (98%), nail changes (93%), melanocytic nevus (78%),seborrheic keratosis (54%), pigmentation changes (51%), uremic pruritus (49%),tinea pedis (34%), actinic keratosis (23%), skin tags (16%), onychomycosis(17%), lentigo (16%) and cherry angioma (11%) were the most commondermatologic lessions.There was not statistically significant relation between uremic prurituswith primary disease, duration of hemodialysis, presence of kserosis, anemia andparathormon levels. But there was a statistically significant relation betweenuremic pruritus with anti HCV antibody existence.General skin diseases had not increase frequency but seborrheic andactinic keratosis, which are the tumoral skin lesions, had increased frequency. Wethought that this is why the most of patients had advanced age.49Nail changes were observed most frequently as longitidunal riddging, halfand half nail, absent of lunula and splinter hemorrhages. Unlike the literaturelongitidunal ridging was observed most frequently. There was not statisticallysignificant relation between nail changes with age, smoking, and primary disease.There was statistically significant relation between genders with half andhalf nail, splinter hemorrhages and terry nails. There was statistically significantrelation between duration of hemodialysis with half and half nails and splinterhemorrhages. There was statistically significant relation between half and halfnails with hemoglobin, albumin, and ferritin levels, between splinter hemorrhageswith hemoglobin levels, between muerchke lines with hemoglobin levels,between absent of lunula with parathormone levels and between beau lines withCa levels.The dermatologic manifestations may be an important guide to thediagnosis of renal disease. A well performed dermatologic examination may be animportant component of the first impression guiding to the diagnosis in the patientwith chronic renal failure.
