Hajiyeva S. The Effect of Adrenal Gland Metastasis on the Course of Lung Cancer. Eskişehir Osmangazi University Faculty of Medicine Department of Chest Diseases. Specialist Thesis in Medicine, Eskişehir, 2024. The aim of this study was to investigate the frequency, characteristics and impact on survival of adrenal metastases in lung cancer patients diagnosed, treated and followed up in our clinic. In the study 1,253 lung cancer patients were analyzed between January 1, 2011 and December 31, 2015. Adrenal lesions were categorized into the groups < 1 cm and ≥ 1 cm at diagnosis. Of the patients with an adrenal lesion of less than 1 cm, 0.7% (7/971) had adrenal metastases at diagnosis and 6% (59/971) during follow-up. Of the patients with an adrenal lesion of 1 cm or more, 54.6% (154/282) had an adrenal metastasis at diagnosis and 45.4% (128/282) had a benign adrenal lesion. The survival time was 12.0 ± 12.5 (11.0 – 13.0) months in patients without metastases at diagnosis, 10.0 ± 3.1 (4.0 – 16.0) months in patients with metastases only in the adrenal gland and 7.0 ± 0.6 (5.9 – 8.1) months in patients with metastases in the adrenal gland and other organs. While the survival time of patients without metastases was similar to the survival time of patients who only had metastases in the adrenal gland (p=0.119), the survival time of both patients without metastases and patients who only had metastases in the adrenal gland was longer than that of patients who had metastases in other organs in addition to the adrenal gland (sırasıyla p<0.001, p=0.002). On multivariate analysis, the determinants of adrenal metastases at diagnosis were nodular structure of the adrenal lesion (OR; 95%CI: 4.97; 2.10-11.87), SUVmax value ≥ 2.5 (5.57; 3.25-9.53), brain (OR: 3.24; 1.17-8.18), bone (3.87; 1.59-9.43) and pleural metastases (2.55; 1.02-6.38). The variables that predicted malignant or benign adrenal lesions were adrenal lesion size (2.98; 1.33-6.67) and SUVmax (8.25; 5.00-13.61).
